If you wish to submit digitally, please first download each document and then use Adobe Acrobat Reader (available for free) to fill out the forms and e-sign them. Once completed, please save them and then email them to forms@fertilitycenterdls.com.
It is also possible to print and complete each form and then fax them to 214-887-8244.
